Become an Associate Member

Everyone who cares about the Chilterns MS Centre is warmly invited to become an Associate Member.  It’s a great way to demonstrate your personal commitment and support for the Centre’s work helping people with Multiple Sclerosis.  It also makes it easy to keep in touch with all the latest news from the Centre, including notice of events and special occasions. 

Membership benefits include

• 2 Centre Newsletters a year

• 6 eNewsletters a year

• A smart pin badge for you to wear

• Information about forthcoming fundraising and social events in support of the Centre.

• Your personal copy of our Annual Review

• A personal invitation to our Annual General Meeting (please note that Associate Members are NOT entitled to vote). 

An annual donation

Annual membership is a suggested minimum donation of £15.00 and the membership year runs from September to August.  If you are a tax payer, we will be able to Gift Aid your donation. 

What happens next?

Please complete an application form.  Choose from an interactive form that can be completed online and returned to us by email or download a standard form to print out and return to us.

If you wish, you can choose to make a regular donation by completing the standing order form to maintain your Associate Membership.  Click here for the Interactive Standing Order Form or the standard form

If you are a tax payer, please complete the Gift Aid Declaration too.  Either the interactive version or the standard version.
